Medical Forms
National policy requires that all adults and children who attend a Scouting overnight have on file with the Pack a physically printed and signed hardcopy of Parts A, B1, and B2 of the Annual Health & Medical Form (AMHR), commonly known as then “med forms”.
Med forms are good for 12 months. Turn in new/updated med forms for all members of your family to the designated Medic or a Pack leader upon arrival at your first campout or overnight each year. Or you may be requested to turn in med forms at a Pack meeting before the overnight to reduce the paperwork upon arrival at the overnight.
Tips:
Don’t forget to sign and date page A, and if you wish to authorize administration of non-prescription meds for common ailments also sign and date page B2.
Include a printed photocopy of the current health insurance card for each person.
When turning in med forms for an adult participant, also turn in a hardcopy of the adult’s SYT completion certificate.
Make copies of the med form PDF and fill in each one for the different members of your family. These copies can be saved, updated, and printed as needed, which is much easier than starting from scratch each year.
If you forget to bring med forms to a Scouting overnight, you will be asked to fill them out when you get there. It takes a while, so it is not fun for you (or us) when this happens. If possible, make sure you bring the med forms already completed.
Scouting America discourages Scouting units (packs and troops) from storing med form files electronically because of the sensitive health information they contain. Med forms on-file with Pack 421 are physically destroyed when no longer needed.
If you come across Part C of the AMHR, know that it is for long-term and high adventure trips and not typically required for events at the Cub Scout level.
